51 year old female client, slightly overweight, suffers fatigue due to stress. She finds it hard to fall asleep at night and also not feeling refreshed when waking up. She also suffers swelling in both ankles and lower back pain if she walks too far.

Therapist point of view:
- Blood & Qi deficient, possibly some Qi stagnation and dampness.
- Tonify Heart for blood and to improve sleep.
- Tonify Spleen for building qi.
- Clear dampness for better qi circulation
Left image: Before treatment - Colour of tongue seems normal. Slight yellow coating - internal coldness. On the edge of the tongue seem like a lump which may indicate Qi stagnation.
Right image: After treatment - Colour of tongue seems more vibrant - more blood in the body. Coating on the tongue minimized meaning the internal coldness has normalised. The general shape and the lump on the edge of the tongue has transformed. Tongue looks more proportional and qi is flowing better.

Last week I worked on a 12 year old patient, suffering from nightmares and occasional bed-wetting. As a therapist and also a traditional chinese medicine practitioner, I tried to look at all aspect of his life to get a better understanding of one's internal organ disharmonies. There are also issue with him and his school teacher, plus he feels insecure in his family as his parents are separated. His initial complexion is quite pale and baggy eyes; signs of exhaustion.
Complaints: Headache, lower back pain, restlessness/irritability, short attention spam.
From chinese medical practitioner point of view, for one to experience nightmares and insecurity comes from disharmony in the heart and possibly disharmony in the stomach. His bed-wetting show signs of kidney energy being weak; the cause is also coming from the insecurity he feels at home.

- Improve his sleep - improve sleep can calm the mind and replenish energy; worked on heart channel and neck massage.
- Better diet - having nightmares can be cause by stomach heat rising. The patient also consumes a lot of fatty and greasy food. More veges!
- Stimulate Kidneys - Kidneys are known to be the essence of life. To stimulate it can revitalise energy. Lower back massage.
- Clear Stomach Heat - regulate the stomach channel to stop energy rising to the head. Massage legs to draw energy down.
Above are the patient's tongue. In traditional chinese medicine, the tongue provides signs of disharmony.
Overall - Treatment was executed to improve:

- Crack in the middle - this is known as Kidney and Stomach Yin deficiency. After treatment the crack has slightly minimized at the tip.
- Shape - noticed before treatment the tongue is not full; half of the tongue is flattened = qi (energy) is depleted. After treatment the tongue became full proportionally.
- Colour - Red in colour show signs of heat. Noticed after treatment the colour became pink.
- Coating - to have a thin layer of white coating on the tongue is normal. Before treatment there was no coating = yin deficient. After treatment due to calming the mind, yin is slightly restored.
Overall - Treatment was executed to improve:

A 58 yr old female client came to me complaining of tiredness due to lack of quality sleep; her sleep is mainly broken. She also suffers from nightmare or great deal of dreams. Client also works night shifts. Recent last couple of days she has been drinking a lot. Her stress level is high due to family issues.

Therapist point of view:
- Disturb sleep is due to Heart energy is not able to put the mind to rest. In TCM, the heart houses the mind. If we have trouble sleeping it is usually heart is in disharmony.
- I also noticed the client's muscles are tight. Good healthy muscles are sponge-like and fleshy. Spleen governs the muscle in TCM. With long term stress and worries can cause spleen to work inefficiently, developing poor quality muscles. When muscles are tight, qi/energy will be restricted.
- Client's complexion seems droopy. Possibly suffering from exhaustion.
Treatment Plan:
- Improve sleep - Regulate Heart channel to put the mind at ease. Massage feet, neck and head.
- Clear Heat - stress, lack of sleep plus alcohol stimulates the body.
- Build Qi and improve circulation - stress blocks energy flow. Liver governs qi flow & spleen governs the transformation from food and air into qi. Stimulate the liver channel and spleen channel will build energy.

- Crack at the tip of tongue shows heart disharmony. The sleepless nights seems to be chronic.
- Colour - pale; not enough blood.
- Shape - swollen. Dampness in the body due to spleen disharmony. Teeth marks at the edges > deficient in qi
- Crack is less prominant
- Red in colour due to increase of blood
- less prominant teeth marks - qi has been nurtured.
